Masculinizing Mastectomy (F-M)

What is it?

F-M masculinizing mastectomy techniques are similar to those used to treat gynecomastia. Surgery usually combines liposuction with direct excision of glandular tissue, which is generally firmer and more difficult to aspirate, and reduction of the areolas.

When the breast is larger, some skin may also need to be removed. In these cases, a scar along the inframammary fold may be required.

Breast Augmentation with Implants (M-F)

What is it?

Breast augmentation uses silicone implants to change breast shape and volume. Planning depends on height, chest shape, tissue thickness and quality, and the amount of pre-existing breast tissue. Over time, loss of tissue elasticity, physical trauma, weight changes or pregnancy may alter breast shape and lead to revision surgery.

Implant shape and volume. An anatomical implant may adapt to the pre-existing breast and create a balanced contour; in some patients, a round implant may achieve an equally harmonious result. Volume is selected according to chest and breast measurements. In general, larger implants may increase the risk of complications and reduce the natural appearance over the medium or long term.

Implant plane. The implant may be placed in a prepectoral plane, in front of the muscle, or using a dual-plane technique, with the upper pole behind the muscle and a variable extent of the lower portion in a prepectoral position. Dual-plane placement can improve coverage in patients with little breast tissue, reduce implant visibility and rippling, and allow better lower-pole expansion. A prepectoral plane may be considered when there is enough breast tissue to conceal the implant or in patients undertaking very intensive sport, avoiding implant movement with pectoral muscle contraction.

Incision and scar. The most common approaches are inframammary, periareolar and axillary. The choice depends on anatomy, implant volume and procedural safety. A large implant may not pass through a periareolar approach when the areola is very small; the axillary approach offers less internal visualisation and has a theoretically higher bleeding risk.

Mentoplasty with Implant (F-M)

What is it?

Chin implants are solid and shaped to improve chin contour and volume. They may be made of silicone or MEDPOR.

Through an incision inside the mouth or below the chin, the implant is placed next to the bone and fixed with screws or sutures. Compared with hyaluronic acid or fat grafting, it can provide sharper definition and a permanent result.

Hyaluronic Acid Fillers

PurposeTemporarily restores or increases facial volume, improves projection and helps correct wrinkles.

What is it?

Filler injection is a non-surgical procedure that uses filling substances to improve wrinkles, folds, irregularities and volume. In clinical practice, non-permanent hyaluronic acid is preferred because of its safety profile.

Well-known brands include Restylane, Perlane and Juvederm. Products differ in viscosity, water absorption and molecular characteristics, which influence injection depth and durability. They may be used in the cheeks, folds, lower eyelid–cheek transition, lips, chin and nose.

In transgender treatments, judicious placement can help define more masculine or more feminine facial features.

What is a masculinizing mastectomy?

What scars may result from surgery?

A scar may be placed around the areola. Larger breasts may require an additional scar along the inframammary fold.

What is recovery like after a masculinizing mastectomy?

Return to work is usually possible after about two weeks if no exertion is required. Full arm elevation starts from the second week, exercise after six weeks, and chest compression is recommended for six weeks.

What are the main risks, and is the result permanent?

Swelling, bruising, seroma, sensory changes, haematoma, infection, excessive scarring, pigmentation, irregularities or adhesions may occur. The procedure is usually considered definitive.

How is breast augmentation with implants planned?

Planning considers height, chest and breast dimensions, nipple position, tissue thickness and quality, and the amount of pre-existing breast tissue.

How are implant shape and volume selected?

An anatomical or round shape and the implant volume are selected according to anatomy and the desired result. Larger implants may increase complication risk and reduce the natural appearance over the medium or long term.

Where can a breast implant be placed?

What are the anaesthesia, hospital stay and recovery?

Surgery may be performed under general anaesthesia or deep sedation with an anaesthetist, as an outpatient procedure or with one day in hospital. Return to work is usually possible in five to seven days without exertion, and intense exercise after six weeks.

Do breast implants last a lifetime?

They should not be considered lifetime devices. Revision, removal or replacement may be required over time, particularly when surgery is performed at a young age.

What are the main implant complications?

Complications include pain, swelling, haematoma, seroma, infection, sensory changes, unfavourable scarring, asymmetry, malposition, rotation, rippling, palpability, capsular contracture, rupture and further surgery.

What is capsular contracture?

It is excessive tightening or hardening of the scar capsule that naturally forms around an implant. It may cause firmness, pain and breast shape changes and can require surgery.

What is silent implant rupture?

A silicone implant rupture may not cause obvious symptoms. When clinically suspected, ultrasound or magnetic resonance imaging can assist assessment.

What can happen if an implant moves or develops rippling?

Malposition or rotation can change breast symmetry and contour. Rippling refers to visible or palpable folds and is more likely when soft-tissue coverage is limited.

What is BIA-ALCL?

It is a rare T-cell lymphoma that usually develops in the fluid or capsule surrounding a breast implant. It is not carcinoma of the breast tissue.

What are the signs of BIA-ALCL, and how is it treated?

The most common signs are late persistent breast enlargement, seroma, a mass, pain or firmness. Assessment may include clinical examination, imaging and analysis of fluid or tissue. Localised disease is generally treated by removing the implant and capsule; advanced disease requires multidisciplinary oncology care.

What is chin implant surgery?

A solid implant is placed next to the chin bone to increase projection and define the contour, creating sharper features and a permanent result.

Which materials may be used, and how is the implant fixed?

The implant may be made of silicone or MEDPOR and is secured next to the bone with screws or sutures.

Where is the scar, and what is recovery like?

The incision may be inside the mouth or below the chin. Light work is usually possible after two to three days, although swelling may persist for about three weeks.

What are the risks of chin implant surgery?

Swelling, bruising, asymmetry, infection, capsular contracture, implant displacement and irritation or injury of the mental nerve may occur. Some complications may require implant removal.

How can hyaluronic acid help define gender-related facial features?

Judicious placement may enhance or soften volumes and projection to favour more masculine or more feminine facial features.

How are hyaluronic acid fillers administered?

This is a non-surgical consultation-room procedure in which a non-permanent product is injected at selected depths and facial areas according to the intended result.

What is recovery like after filler treatment?

Work can usually be resumed on the same day. Swelling, redness or bruising may persist for one or two days.

What are the risks, and how long do results last?

Less commonly, asymmetry, irregularities, undercorrection, overcorrection or infection may occur. Results usually last six to eighteen months depending on the product, depth and area; B. Toxin may be combined when clinically appropriate.
